Manual Handling Risk Assessments: Why Training Alone Often Fails

Manual handling is one of the most familiar workplace health and safety topics.

Most businesses know that lifting, carrying, pushing, pulling or moving loads can cause injury. Many have sent staff on manual handling training. Some have posters showing people how to lift correctly. Others have risk assessments, toolbox talks or online training records.

Yet manual handling injuries still happen.

That is because manual handling risk is not solved by training alone.

Training can help people understand risk and use better techniques, but it cannot make a heavy load lighter. It cannot remove an awkward lift. It cannot fix poor storage heights. It cannot redesign a cramped workspace. It cannot eliminate time pressure, fatigue, poor planning or lack of mechanical aids.

If the task itself is badly designed, asking people to “lift correctly” may only go so far.

A good manual handling risk assessment looks beyond the person doing the lift. It looks at the task, the load, the working environment, individual capability and the control measures needed to reduce risk.

What Counts As Manual Handling?

Manual handling is broader than many people realise.

It is not just lifting a box from the floor.

Manual handling can include:

  • lifting
  • lowering
  • carrying
  • pushing
  • pulling
  • holding
  • supporting
  • manoeuvring
  • team handling
  • repetitive handling
  • awkward handling
  • moving people or animals where relevant

The Health and Safety Executive’s guidance on manual handling at work explains that employers must avoid hazardous manual handling where reasonably practicable, assess the risk where it cannot be avoided, and reduce the risk of injury as far as reasonably practicable.

That hierarchy matters.

The first question is not “have people been trained?”

The first question is “can the hazardous manual handling be avoided?”

Why Manual Handling Still Causes Problems

Manual handling risk is often underestimated because it is part of normal work.

People move boxes, tools, furniture, stock, equipment, materials, waste, files, parcels, displays, cleaning supplies and products every day. Because these tasks are routine, they can stop looking like risks.

But routine does not mean safe.

The latest HSE statistics show that 511,000 workers were suffering from a work-related musculoskeletal disorder in Great Britain in 2024/25. These conditions include injuries and disorders affecting the back, neck, upper limbs and lower limbs. The HSE also reports that 40.1 million working days were lost due to work-related ill health and non-fatal workplace injury in 2024/25. The HSE’s health and safety statistics give useful context for why musculoskeletal risk still matters.

Not all musculoskeletal disorders are caused by manual handling. Some relate to repetitive work, awkward posture, display screen equipment, vibration or other work factors. But manual handling remains one of the most common and practical areas where businesses can reduce risk.

The problem is that many businesses focus on the worker’s behaviour rather than the design of the task.

Training Has A Place – But It Is Not A Control By Itself

Manual handling training can be useful.

It can help employees understand how injuries happen, how to plan a lift, how to use mechanical aids, how to recognise high-risk tasks and when to ask for help. It can also make people more aware of posture, twisting, overreaching and poor lifting habits.

But training should not be used as a substitute for proper risk control.

If a task involves repeatedly lifting heavy items from floor level, the main issue is not whether workers have watched a manual handling video. The issue is why the items are stored on the floor, whether the load can be reduced, whether lifting equipment can be used, whether the process can be redesigned, or whether fewer lifts are needed.

If employees have to carry awkward loads through narrow routes, training alone will not widen the route or change the load.

If staff are under time pressure, training may be forgotten or ignored.

If the only available trolley has broken wheels, training will not make it effective.

That is why manual handling risk assessment needs to look at the work system, not just the worker.

The Manual Handling Hierarchy

The most important principle in manual handling is the control hierarchy.

In plain English, the business should consider:

  1. Can the hazardous manual handling be avoided?
  2. If not, has the task been properly assessed?
  3. Can the risk of injury be reduced so far as reasonably practicable?

That order is important.

Avoidance might include using mechanical handling, changing delivery methods, altering storage arrangements, using smaller containers, redesigning workflow or removing unnecessary movement of materials.

Assessment then considers the risk factors that remain.

Reduction might include using trolleys, sack trucks, pallet trucks, hoists, conveyors, lift tables, better storage heights, job rotation, team handling, improved floor surfaces, clearer routes, better planning, training and supervision.

Training is usually part of the third stage. It is not the whole strategy.

The TILE Approach

A common way to think about manual handling risk is TILE:

  • Task
  • Individual
  • Load
  • Environment

Some versions include additional factors such as Equipment or other organisational issues, but the basic TILE approach remains useful.

It helps prevent businesses from focusing only on load weight.

A load may be light but still difficult if it is awkward, unstable, hard to grip or handled repeatedly. A task may be manageable once but risky when repeated many times. A lift may be safe for one person but unsuitable for another. An environment may make an otherwise simple task hazardous because of poor space, lighting, floor condition or access.

A good assessment looks at the full picture.

Task: What Is The Person Actually Doing?

The task is often the most important part of the assessment.

You need to look at how the manual handling is actually carried out, not how it is supposed to happen in theory.

Key questions include:

  • Is the lifting frequent or repetitive?
  • Is the load lifted from floor level?
  • Is the load lifted above shoulder height?
  • Is the worker twisting while lifting?
  • Is the load carried over a distance?
  • Is pushing or pulling involved?
  • Is the task rushed?
  • Is team handling used?
  • Is the movement awkward or restricted?
  • Is the task carried out at the end of a long shift?
  • Are there sudden or unexpected movements?
  • Does the task involve people, animals or unstable items?

Repetition is often overlooked.

A single lift may seem harmless. Repeating the same movement hundreds of times can create a very different risk.

The same applies to posture. A moderately heavy load held close to the body at waist height may be manageable. The same load lifted from the floor, twisted sideways and placed on a high shelf may be much more risky.

Individual: Not Everyone Has The Same Capability

Manual handling risk depends partly on the individual.

People vary in height, strength, fitness, experience, age, health, training, fatigue, pregnancy, disability and previous injury.

That does not mean risk assessments should be based on the strongest person in the workplace. In fact, that is often a mistake.

If only the strongest or most experienced worker can complete a task safely, the task may not be suitable for the wider workforce.

A proper assessment should consider the range of people likely to do the work.

New workers may not understand the best method. Temporary staff may not know where equipment is stored. Younger workers may lack experience. Older workers may have different physical limitations. Someone returning from injury may need adjustments. Pregnant workers may need additional consideration.

The aim is not to make assumptions about people. The aim is to design tasks so they are reasonably safe for those expected to carry them out.

Load: Weight Is Only One Factor

Businesses often ask, “what is the maximum weight someone can lift?”

That is usually the wrong starting point.

There is no simple safe weight that applies in all circumstances. Risk depends on the load and the way it is handled.

The load may be risky because it is:

  • heavy
  • bulky
  • awkwardly shaped
  • unstable
  • difficult to grip
  • sharp or hot
  • wet or slippery
  • likely to shift suddenly
  • carried away from the body
  • blocking the worker’s view
  • handled repeatedly
  • unevenly weighted

A lighter load may create more risk than a heavier one if it is awkward, unstable or lifted in a poor posture.

Good manual handling assessment therefore looks at more than weight. It considers how the load behaves and how it affects the worker’s posture and control.

Environment: Where The Handling Takes Place

The working environment can turn a simple manual handling task into a risky one.

Factors may include:

  • uneven floors
  • wet or slippery surfaces
  • steps or slopes
  • poor lighting
  • narrow routes
  • clutter
  • poor housekeeping
  • temperature extremes
  • restricted headroom
  • poor access to storage
  • long carrying distances
  • vehicle loading areas
  • lack of space around the load
  • unsuitable racking or shelving

For example, carrying boxes across a clear, level, well-lit floor may be relatively straightforward. Carrying the same boxes down steps, through a narrow doorway, across a wet yard or around stored materials is a very different task.

Many manual handling improvements come from changing the environment rather than changing the person.

Better storage, clearer routes, improved flooring, better lighting and suitable handling aids can all make a significant difference.

Mechanical Aids Must Be Suitable And Available

Mechanical aids can reduce manual handling risk, but only if they are suitable, available and used properly.

Examples include:

  • trolleys
  • sack trucks
  • pallet trucks
  • lift tables
  • hoists
  • conveyors
  • scissor lifts
  • drum handlers
  • vacuum lifters
  • adjustable height benches
  • powered handling equipment

A common problem is that equipment exists but is not used.

That may be because it is stored too far away, difficult to access, poorly maintained, unsuitable for the route, too slow, not charged, damaged, or not understood.

If employees regularly choose not to use a manual handling aid, the business should ask why.

Simply telling people to use the aid may not be enough if the aid does not work well in practice.

A proper assessment should consider whether equipment is suitable for the task and whether the workplace supports its use.

Team Handling Is Not Automatically Safe

Team handling is often used when a load is too heavy or awkward for one person.

It can reduce individual effort, but it can also introduce new risks.

Team handling may create problems where:

  • people are different heights
  • communication is poor
  • movement is not coordinated
  • the route is restricted
  • one person takes most of the load
  • the load is unstable
  • visibility is poor
  • the team has not agreed who leads
  • people have different levels of strength or experience

Team handling should therefore be planned.

It is not enough to say “use two people”. The assessment should consider how the lift will be coordinated, whether the route is suitable, and whether the people involved understand the method.

Sometimes a mechanical aid is a better solution than adding more people.

Storage Heights Matter

One of the simplest ways to reduce manual handling risk is to improve storage.

Heavy or frequently used items should normally be stored at a height that avoids lifting from the floor or reaching above shoulder level.

Poor storage creates unnecessary risk.

Common issues include:

  • heavy items stored too high
  • heavy items stored too low
  • frequently used items stored in awkward places
  • shelves overloaded
  • stock rotated poorly
  • items stored where access is restricted
  • unstable stacking
  • lack of space to get close to the load

Good storage design can reduce bending, twisting, reaching and carrying distance.

This is often a low-cost improvement compared with dealing with injuries, absence and disrupted work.

Pushing And Pulling Risks Are Often Missed

Manual handling is not only lifting and carrying.

Pushing and pulling can also create risk, especially where trolleys, cages, pallet trucks, wheeled bins or equipment are moved.

Risk factors include:

  • heavy loads
  • poor wheels
  • uneven floors
  • slopes
  • tight turns
  • poor visibility
  • poor handles
  • sudden stops
  • long distances
  • poor maintenance
  • congested routes
  • poor footwear
  • lack of braking control

Pushing is generally preferable to pulling, but that does not automatically make it safe.

The condition of the equipment and route can make a huge difference. A well-maintained trolley on a smooth, level surface may be easy to move. A poorly maintained trolley over rough ground may require excessive force.

The assessment should consider the real conditions, not just the existence of wheeled equipment.

Repetitive Handling And Cumulative Strain

Manual handling injuries are not always caused by one heavy lift.

They can also develop through repetition.

Repeated lifting, reaching, twisting, gripping, pushing or pulling can create cumulative strain over time.

This is common in:

  • warehousing
  • packing
  • production lines
  • retail stock work
  • care work
  • cleaning
  • food production
  • delivery work
  • laundry work
  • workshop tasks
  • order picking

The task may not look dangerous at first glance. The issue is the number of repetitions, the pace, the posture and the lack of recovery.

Controls may include redesigning the task, reducing repetition, rotating work, using mechanical aids, changing layout, improving work height or adjusting staffing and workload.

Again, training alone is unlikely to solve a repetitive design problem.

Manual Handling In Offices

Manual handling is often associated with warehouses and industrial workplaces, but offices can also have risks.

Examples include:

  • moving boxes of files
  • relocating furniture
  • handling deliveries
  • moving IT equipment
  • lifting water bottles
  • handling paper supplies
  • carrying display materials
  • setting up meeting rooms
  • archiving documents
  • clearing storage areas

Because offices are seen as low risk, these tasks may be done informally.

Someone may lift a heavy box because “it will only take a minute”. Staff may move furniture without planning. Deliveries may be carried up stairs. Archived files may be stored in awkward locations.

Office manual handling does not need to be overcomplicated, but it should not be ignored.

Simple controls such as smaller boxes, trolleys, better storage, asking for help, avoiding overloaded containers and planning moves properly can prevent problems.

Manual Handling In Warehousing And Manufacturing

Warehousing and manufacturing environments often have more obvious manual handling risks.

These may include:

  • loading and unloading
  • palletising
  • order picking
  • production line handling
  • packing
  • moving raw materials
  • handling waste
  • machine feeding
  • tool changes
  • drum or container handling
  • repetitive lifting
  • pushing cages or pallet trucks
  • working around vehicles

The risk may also increase during peak periods, staff shortages, temporary storage changes or when agency workers are used.

In these environments, manual handling should be considered alongside workplace transport, racking, pedestrian routes, machinery safety and production planning.

A manual handling assessment that ignores workflow is unlikely to be effective.

Manual Handling And Contractors

Contractors may introduce manual handling risks or be affected by your site arrangements.

For example, contractors may bring materials onto site, move equipment through your premises, carry tools, install plant, deliver stock or remove waste.

If your business controls the site, you need to think about how contractor activities interact with your workplace.

Are routes clear? Are lifts available? Is storage agreed? Are there restrictions on delivery times? Is there a safe unloading area? Are contractors bringing equipment that creates risk to employees?

Equally, where your employees work on client sites, they may face manual handling conditions outside your direct control.

Contractor and client-site arrangements should therefore be considered where relevant.

The HSE MAC Tool

For lifting, carrying and team handling tasks, the HSE’s Manual Handling Assessment Charts, known as the MAC tool, can help businesses assess common risk factors.

The HSE explains that the MAC tool is designed to help users understand, interpret and categorise the level of risk in lifting, lowering, carrying and team manual handling activities. It uses colour coding and scoring to highlight risk factors and help identify higher-risk tasks.

The MAC tool is not needed for every minor task, but it can be very useful where manual handling is frequent, repetitive, heavy, awkward or difficult to judge.

It helps structure the assessment and makes it easier to see which factors are increasing risk.

For example, the issue may not be just the weight of the load. The tool may highlight carrying distance, hand distance from the lower back, twisting, grip, floor surface or other factors.

That can make improvements more targeted.

Common Manual Handling Failings

Manual handling failings are often very practical.

Common examples include:

  • relying on training alone
  • failing to avoid hazardous manual handling where possible
  • no task-specific assessment
  • assessments that focus only on weight
  • heavy items stored at poor heights
  • poor access to loads
  • lack of suitable handling aids
  • handling aids provided but not used
  • poor maintenance of trolleys or pallet trucks
  • excessive carrying distances
  • awkward twisting or reaching
  • poor housekeeping
  • rushed work
  • lack of planning for deliveries
  • weak supervision
  • no review after injuries or near misses
  • agency or temporary staff not properly instructed
  • team lifting used without coordination

These issues are rarely difficult to understand. The difficulty is often that nobody has looked closely enough at how the work is really being done.

What A Good Manual Handling Assessment Should Cover

A good manual handling assessment should be specific to the task.

It should consider:

  • what is being moved
  • how often it is moved
  • how far it is moved
  • where it is lifted from and to
  • whether bending, twisting or reaching is involved
  • whether the load is heavy, awkward or unstable
  • whether the worker can get close to the load
  • whether mechanical aids can be used
  • whether the route is clear and suitable
  • whether team handling is needed
  • whether employees have suitable training
  • whether individual capability affects the task
  • whether work pace or staffing levels increase risk
  • what further controls are needed
  • who is responsible for action

The assessment should lead to practical improvements.

If the answer is always “staff trained”, the assessment is probably too weak.

What Businesses Should Do Next

If you are unsure whether your manual handling arrangements are suitable, start with the tasks people complain about, avoid, rush or find physically difficult.

Ask:

  • can the manual handling be avoided?
  • can the load be made lighter or smaller?
  • can the task be redesigned?
  • can mechanical aids be used?
  • are heavy items stored at sensible heights?
  • are routes clear and suitable?
  • are trolleys or handling aids maintained?
  • are staff lifting from the floor or above shoulder height?
  • is the work repetitive?
  • are employees twisting, reaching or rushing?
  • are temporary or agency workers involved?
  • has anyone reported pain, discomfort or near misses?
  • does training reflect the actual task?

Do not start by assuming the answer is more training.

Start by asking whether the task itself can be improved.

Final Thought

Manual handling training has value, but it is not enough on its own.

If the task is poorly designed, the load is awkward, the route is difficult, the equipment is unavailable, or the work is rushed, training will only have limited effect.

A proper manual handling risk assessment looks at the whole system.

It asks whether the handling can be avoided, whether the risk has been properly assessed, and whether practical changes can reduce the chance of injury.

That might mean better storage, better equipment, better planning, better supervision, better task design, or better use of mechanical aids.

The aim is not to teach people to cope with poor manual handling conditions.

The aim is to make the conditions better.
